I understand that the application or removal of permanent makeup is an elective cosmetic procedure and that results may vary. I acknowledge that multiple sessions may be required to achieve my desired results and that outcomes cannot be guaranteed. I understand that some discomfort, swelling, redness, or bruising may occur, and that pigment color may fade or change over time due to factors such as skin type, sun exposure, skincare products, medications, or other treatments.

I confirm that I have disclosed all relevant medical conditions and medications and that I am in generally good health. I understand that certain skincare products, laser treatments, or cosmetic procedures such as Botox or dermal fillers may affect the appearance or longevity of my permanent makeup. I agree to inform any medical or skincare professional that I have permanent makeup before undergoing related treatments.

I understand that proper aftercare is essential to healing and results, and I agree to follow all aftercare instructions provided to me. I accept responsibility for communicating my preferences for color, shape, and placement and for approving the final design before the procedure begins.

I acknowledge that risks may include but are not limited to skin reactions, infection, pigment migration, poor retention, or hyper/hypo-pigmentation. I understand that future touch-ups may be required and are not included unless otherwise stated. I understand that I may request a patch test but may choose to waive this option and acknowledge that allergic reactions can occur at any time.

I consent to photographs being taken for legal documentation and marketing purposes unless otherwise requested. In the unlikely event of a needle-stick injury to my practitioner during the procedure, I agree to cooperate with appropriate medical testing for their safety. I understand that if signs of infection or adverse reactions occur after my procedure, I should seek medical attention promptly.
